MONTHLY MEMBERSHIP AGREEMENT
You are electing to join the Ellevate Med Spa Monthly Membership program by which you will make monthly payments into your Ellevate Med Spa Wallet for the rights to receive discounted rates on certain services provided by Ellevate Med Spa. Please check the Membership-program you are electing to participate in pursuant to the terms and conditions provided in this Agreement.
The Ellevate $99 Membership ( 12 Months )
*OVER ($1,000) in annual savings
Payments of $99 per month to your Ellevate Med Spa Wallet, which may be applied towards any Treatment provided by Ellevate Med Spa at the time of that Treatment at the following discounted rates (any Treatment not listed is available, but will be at our normal pricing at the time of Treatment):
UNLIMITED USE
- $14/unit Xeomin (usually $15/unit)
- $14/unit Botox (usually $15/unit)
- $14/ (equivalent dosing) Dysport (usually $15/unit)
- 10% OFF Ellevate Medical Grade Skin Care
- 15% OFF ALL Vitamin Injections (normally $40 - $50 per injection)
PLUS YOU MAY CHOOSE 1 DISCOUNTED SERVICE PER MONTH
- $50 OFF LaseMD (Laser Skin Rejuvination and Tightening) (normally $550)
- $50 OFF Genuis RF Microneedling (normally $750 per session)
Payment on any Treatments beyond Your Ellevate Med Spa Wallet balance will be payable in full at the time of service.
